Pay Range: $17.50 - $19.50

RESPONSIBILITIES & DUTIES

The Community Support Specialist is the direct service provider who may be responsible for but not limited to the following essential duties:

  • Implement individual training and support programs in the areas of post-secondary education, pre-employment, volunteering, and social/recreation services under the direction of the case responsible person.
  • Develop, secure and coordinate appropriate community resources and settings that will develop the individual skills sought by our clients.
  • Provide activities of social/behavioral value in the community including education in anger management, communication skills, symptom monitoring, etc., to support the individual in maintaining behavior appropriate to the activity or site.
  • Maintain positive and professional communications and relationships with his/her supervisor, clients, employers, involved members of families and other individuals in the community.
  • Participate in the evaluation of the client through completion of task analysis and other requested assessment devices.
  • Provides logs, reports, and client evaluations in a timely, complete, and accurate manner as required.
  • Assist in ensuring the safety of the clients and staff during all activities.
  • Transport clients to appointments, community activities, or other locations as needed, ensuring safety and timeliness during travel.
  • Perform all duties of a direct service provider as assigned.

MINIMUM QUALIFICATIONS
  • 18 years of age or older
  • Must have demonstrated knowledge and experience in navigating through post-secondary educational processes such as enrolling in college, accessing educational and institutional resources and completing requirements associated with successful educational experience.
  • Must have one year of experience in a Social Service setting, assisting disadvantaged, distressed, or vulnerable persons or groups.
  • Ability to follow instructions is critical to this position.
  • Ability to multi-task, work under pressure and manage time is also an essential component of this position.
  • Demonstrated ability to effectively navigate and maintain professional boundaries as custodian of confidential information.
  • Certification in an approved First Aid and CPR class; or successfully complete an approved First Aid and CPR class within 45 days of the date of hire.
  • Insured, registered vehicle in safe working condition, and proof of current driver's license.
  • Adjoin's client case management platform requires employees to utilize a mobile smart device (i.e. cellphone/tablet) with reliable internet connectivity and data to complete certain essential job tasks. This position requires the employee to have a mobile smart device available to perform certain required essential functions, for which reasonable reimbursement is provided to the employee.

POSITION REQUIREMENTS
  • Ability to travel within and throughout the counties in which services are provided a minimum of 75% of the time.
  • Ability to travel in and throughout the state as needed to attend meetings, trainings, etc.
  • Successful completion of pre-employment drug screen and background check.
  • Physical evaluations may be required dependent upon specific assigned duties or compliance requirements and may include a TB test and/or a lift and transfer test.
  • Acceptable DMV record.
