Description
JOB SUMMARY
Provides assistance to residents with their routine activities of daily living in accordance with the
company's established procedures.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
• Ensure that full care is delivered to the resident per the care plan. Such care shall include
assisting residents with the following activities of daily living (ADL's): Grooming, Bathing,
Dressing, Eating, Toileting, Incontinent Care, Transfers, Escorting to meals and activities.
• Empty catheter and colostomy bags per training received to perform the task.
• Use of mechanical devices used to transfer residents when needed such as Hoyer lift.
• Interact with family members and medical professionals.
• May be called on to physically escort a resident to a medical appointment outside the facility.
• Recognize and understand signs and symptoms of dementia and respond in accordance with
training received.
• Monitor supplies and reporting needs to LVN Supervisor.
• Under direction of LVN Supervisor and/or Assistant Director of Resident Care, records resident
weight on a monthly basis and records results.
• Ensure that charts, reports and other documentation occurs on a daily basis and is accurate and
thorough.
• Report and write incident reports as needed.
• Assist with set up and clean-up of activities as needed.
• Attend and participate in scheduled training and educational programs.
• Punctual and regular attendance.
• High regard for excellent customer service given at all times.
• Be compassionate, caring, patient and understanding of the needs of the elderly.
• May be required to work beyond scheduled hours.
• Perform other duties as assigned.
Qualifications
QUALIFICATIONS
• Must be at least 18 years of age.
• Must have First Aid Certification.
• Must be able to read, write and speak English fluently.
• Proficient computer skills; must be comfortable charting care, referring to electronic files, and
reporting incidents on the computer.
• Ability to work well with others.
• Enjoy working with senior citizens.
• Ability to take directions from supervisor and follow through to completion with task.
• Ability to respond effectively in an emergency or crisis situation.
• Must have schedule flexibility and be able to work weekends and holidays.
PHYSICAL REQUIREMENTS
This position requires the ability to bend, squat, stoop, kneel, push, pull, sit and stand on feet for
extended periods of time, as well as the ability to lift and carry up to 50 pounds.
