About the Role

The Assistant MDS Coordinator (LVN) plays a vital role in supporting the comprehensive assessment and care planning process within our skilled nursing facility. This position assists in coordinating and completing Minimum Data Set (MDS) assessments to ensure accuracy, timeliness, and compliance with regulatory standards.

Working closely with the interdisciplinary team, the Assistant MDS Coordinator gathers and reviews clinical data, monitors resident status, and helps maintain accurate documentation. This role contributes to ensuring that resident assessments reflect current health needs supporting both quality outcomes and regulatory compliance.

Responsibilities

  • Assist with the preparation, coordination, and completion of MDS assessments in compliance with federal and state regulations.
  • Collect and review clinical data from nursing staff, therapists, and other team members to ensure accuracy and completeness.
  • Collaborate with the MDS Coordinator and care team to develop and update individualized care plans.
  • Monitor resident changes and communicate updates promptly to the MDS Coordinator and clinical staff.
  • Ensure timely submission of MDS documentation to support compliance and reimbursement requirements.
  • Participate in quality assurance activities related to MDS accuracy and compliance.
  • Support and educate staff regarding MDS documentation and related processes.
Minimum Qualifications
  • Current and valid Licensed Vocational Nurse (LVN) license in the state of California.
  • Basic understanding of MDS assessment processes and long-term care regulations.
  • Minimum of 1 year of clinical nursing experience, preferably in a skilled nursing or long-term care setting.
  • Strong organizational skills, attention to detail, and time management abilities.
  • Ability to work collaboratively within an interdisciplinary healthcare team.
Preferred Qualifications
  • Experience assisting with or completing MDS assessments.
  • Familiarity with electronic health record (EHR) systems and MDS software.
  • Additional certifications such as Certified MDS Coordinator (CMS) or Geriatric Nursing Certification.
  • Knowledge of Medicare and Medicaid reimbursement processes.
  • Excellent written and verbal communication skills.
