Day Per Diem Quality Home Health & Hospice Registered Nurse

Do you have a passion for improving quality in healthcare systems? Our established Home Care Agency is seeking a motivated performance-driven nursing professional to join our team. We serve a diverse client base throughout the Greater Sacramento Area.

The Quality Management Nurse is responsible for educating staff and leading performance initiatives. Our QM Nurse will assist in the promotion and maintenance of high quality Home Health patient care and documentation in accordance with Federal and state regulations and The Joint Commission standards. The Quality Management Home Health / Hospice RN position has the opportunity to work remotely about 80% of the time with occasional office meetings and responsibilities. Or if the candidate prefers this position can be office based.

  • Possesses knowledge of appropriate care related to age and disease process as well as documentation requirements for home care.
  • Assists agency in meeting corporate compliance initiatives 100% of the time by completing compliance audits on time and submitting reports to supervisor. Informs Supervisor of identified deficiencies ASAP and assists in designing action plans for any identified deficiencies, as appropriate.
  • Completes assigned projects and corresponding reports within established time frame 90-95 % of the time. This includes the development of the data collection methods as well as assisting in analyzing findings.
  • Demonstrates ability to review clinical charts and computer documentation for completeness and adherence to MHH/MH documentation standards at the rate of 2-3 charts per hour 90-95% of the time.
  • Participates in activities related to the agency meeting regulatory and accreditation requirements by attending 85-90% of the QM meetings annually.
  • Actively participates and/or initiates 1-2 Rapid Cycle projects for OBQI, OBQM, or identified audit deficiencies annually.
  • Actively participates in 3 CQI projects or 3 Utilization Reviews / year.

Job Requirements

The Quality Management Home Health / Hospice RN position has the opportunity to work remotely about 80% of the time with occasional office meetings and responsibilities. Or if preferred this position can be office based.

Required:
  • Current CA Registered Nurse License
  • 2 years Home Health or Hospice experience within the last 5 years
  • Knowledge of State and Federal Regulations and Joint Commission Standards
Preferred:
  • BSN degree and Public Health Certificate preferred.
  • AHA BLS preferred
