Position Title: Patient Care Coordinator

The Patient Care Coordinator plays a vital role in ensuring comprehensive care for individuals with diverse needs. This position involves coordinating appointments, managing travel logistics, and collaborating with primary care providers and clinical teams. The coordinator acts as a central point of contact, guiding patients through their healthcare journey while connecting them to community resources. Salary Range: $52000 / Year

Patient Care Coordinator Responsibilities:

• Support approximately 50 patients at any given time
• Coordinate care, appointments, and transportation for patients
• Engage with patients monthly to assess their conditions and goals
• Document and follow up on patient assessments and care plans
• Collaborate with clinical support teams as needed

Patient Care Coordinator Requirements:

• Minimum of 1 year of experience in the healthcare industry
• Healthcare experience is required
• Reliable transportation and a valid California driver's license are required
• Proficiency in Microsoft Office Suite, including basic formulas in Excel
• A high school diploma or a GED is required
• Bilingual in English/Spanish or Arabic is preferred
• A bachelor's degree is preferred
